Schedule: Full Time Position – Must allow flexibility
according to the needs of the client.
General
Statement of Duties:
The Case Manager provides case management services for children in the
residential program.
Essential Functions:
1.
Promotes the

Plans for the permanency needs of
assigned clients.
3.
Develops and maintains professional and
collaborative relationships with children, parents, community supports, staff,
volunteers, referring agents and other professionals.
4.
Ensures that children receive the
services, which were identified in their service plan.
5.
Monitors the progress of assigned
clients and reports to referring agents.
6.
Develop treatment plan.
7.
Provide family therapy.
8.
Completes documentation in a timely
fashion.
9.
Provide individual counseling and
support to the residents.
10.
Conduct comprehensive assessments,
i.e., psychosocial assessment, social functioning and drug and alcohol
assessment.
11.
Plan for the permanency needs of each
resident.
12.
Participates in the on-call rotation.
13.
Handles unanticipated events and crisis
situations in a positive, constructive manner.
14.
Plans for aftercare of assigned
clients.
15.
Maintains client confidentiality.
Minimum Qualifications:
Candidate must possess a Master’s Degree in
Social Work or a related field. Must possess good written and communication
skills. Intermediate skill level in the use of Microsoft Word, the Internet,
and e-mail. Act 33/34 clearances and
valid driver’s license required.
Additionally, must have use of a car during work hours.
